Those who haven't? I'm sure the list is quite long, since there's hundreds, if not thousands, or heroes between all the companies. But you'd have to know their entire history to know for sure if they haven't, so we won't be able to definitively say too much here.
We might be better off making a list of those who have faced some sort of doppleganger.

Following Secret Invasion most Marvel characters have fought a doppelganger with their powers.
Superman - Several
Green Lantern - At this point thousands but at least Sinestro and Nero
Hulk - Abomination, HKH
Fantastic Four - SuperSkrull
Spiderman - Venom, Carnage, that other one
MartianManhunter - White Martians
Maybe we should have some stipulations.
The hero must've had a solo comic at some point? Or have been part of a big super-hero team, ala Avengers or JSA.
Surfer fought the Keeper and Thor fought a robot clone of himself.

I would say they count if the alternate versions made it INTO the heroes main universe. Joe's counterpart in universe 485 won't count unless it actually made it to his universe and fought 'im.

That's kind of what I was hoping for.
Earthworm Jim got hit by a gun that made opposite copies of it's targets, which basically made "Evil Jim" .... of course Earthworm Jim eventually won by getting hold of this gun and repeatedly shooting Evil Jim with it, thus creating many duplicates of good Jim, all of whom shot the crap out of Evil Jim with their laser guns.
That was cool.
