The thought bomb

This bomb is confusing. I read PoD but it seems to leave many questions unanswered about this power:

1. what are its orgins?
2. does it kill non force sensitives?
3. why does its blast radius seem so small? If it's one of the deadliest weapons ever devised, then shouldn't it be able to devastate, I dunno, an entire planet?
4. what does "trapping their souls side by side for all eternity in an unbreakable state of equilibrium" mean exactly?
