The old janitor is Jake. The woman is not real, or rather she is real but she is based on a woman Jake encountered when he was young in a bar. They were never together. As the janitor he is imagining what it would have been like if he had the courage to approach her and they began to date and he took her to meet his family. He is doing this as he contemplates suicide.

Don't watch it with Walshy though, idk why but I get the feeling he is fidgety during movies. Like he's okay to watch action and comedy movies with, but other than that no.
