This is the answer. The tower has no windows and no doors. It only opens one for either Fate or his wife or someone who has been allowed to enter by them. The only time it has been entered without their consent was in All Star Squadron when Amazingman was still working for Ultra Humanite and was sent to retrieve Fate's helm as one of the artifacts of power he wanted. Even though AM had the power to enter his own molecular structure and phase through the stone walls it took him over an hour to do so. When he went to leave with the helm he could not take it because the tower would not allow the helm to leave even though Fate himself was unconscious. In other tales the tower was even shown to be completely immune to reality warping effects of the likes of Ynar and Vandeamon and protected those within of them too. Even reality warpers can't breach the towers protections. Additionally it is also linked to other realms ( like Strange's sanctum ) and can exist in multiple places ( like when it was linked to the Nelson's brownstone in NY)
