Otto's fist crushed his jaw. Despite it being protected by the armor (doesn't really matter what was stated because all artists portrayed Scorp's helmet like that, it was impossible to punch his jaw off without having to smash through the armor first).
There was also that incident with the vampire Quickshot... when he shot Gargan in the mouth 😂
