He's been in Heaven since his death, but he's felt he still has things left uncompleted, so he never went to Heaven proper; not because he's unable, he just hasn't felt like it. He's literally been sitting on the edge of the floating island and never actually going into the shining city in the center. Then Azazel shows up and now a fight is about to go down.
