Its debate-able. Until we know otherwise however, it is to be assumed that it was actually Vitiate. The game and the Swtor encyclopedia refer to the person you fight as the Emperor and I don't understand why Vitiate wouldn't be personally performing his galaxy-ending ritual.
No, you can only take T3 into that fight. For reasons that I'm not sure if you'd consider spoilers.
It isn't really arguable. Its a rather irrelevant point since even if it was just his Voice, there is no real difference between himself and his Voice, which is stated to 'house the Emperors power and consciousness.'
There are other circumstances too if you'd like me to tell you about them. Basically Vitiate was very weakened, but he had a significant amount of time to recover and was on an extremely powerful Dark Side nexus while the HoT had to fight through a lot of VERY elite troops to get to him.
Since it doesn't specify we can only assume all imo. Other quotes say:
