Okay, so it's a bit of a tough call.

The fear-eater seemed to have some powers, so it's difficult to say whether all of it was an illusion or not. In example it seems like he actually did teleport the Surfer to earth, though I have a hard time imagining a leech could just create a power-cosmic draining device out of thin air.
