Uh....kill her...duh? If he didn't even restrain himself from choking his own wife or fighting the guy who was like a brother to him, why would Ahsoka be anything but dead to him?

I think that if Ahsoka never left the Order she would have gone with Anakin or stopped him from disobeying Mace's orders. They'd never get to knightfall.
Once Anakin killed Mace there was a monumental shift in the force felt by Yoda all the way on Kashyyk. I think Mace's death was certainly part of it, but I think it was also largely because of Anakin's darkness at that point.
