apoc physically handled hulk just fine. given th nature of his powers, he'd get jugg's helmet off at some point and mindrape him ftw more often than not. physical power doesn't really pose much of a threat to apoc anyway. not if he's using his powers correctly as he should in a vs match. /shrug

Apocalypse didn't give the Hulk anything that he hadn't already possessed as seen during the Heart of the Monster. The tech only allowed for the Hulk to focus his power to a degree that he was up until that point unable to do as quickly.

Sword, tentacles, armor. I know, but Apocalypse didn't remake the Hulk, he just gave him a means to reach a height of power that he was unable to gain as easily in all of his history. However, the Green Scar surpassed War's power level. Even when holding back, he was able to slow Cain's advance to a near standstill, and he did it far quicker than War was able to. War was plowed through what seemed to have nearly been a mile before War finally brought him to a near stop.
From what I recall, Cain really didn't push the Green Scar more than a few meters. Anyway, back to the topic. Apoc probably wins this through exploitation considering all of the tricks that he has up his sleeve. Then again you stated that Cain has no weakness to TP these days, so that changes things.... Hmmm, I take it back, Cain probably wins this, or they stalemate until one of them gets bored.
