While I think that you should refer to somebody the way they want to be addressed (just common courtesy), I don't think jail time is an acceptable punishment. That seems a bit disproportionate. You could easily lose your job as I assume bedside manner and positive human interaction is a must in that career field, but jail time is ridiculous.

I'd treat as any other name. It's not like I'm accepting that said person is my master. For all I know, the individual could be the master of something. lol
