Maul got chopped in half and fell down a shaft so Windu getting his hand chopped off and threw out a window seems it's possible.

He's dead. He was supposed to die, there's nothing we can do with him if he survived, and no one really wants him back.

He was also one of the Jedi who could be heard speaking to Rey at the end of RoS.
