Holes in The Matrix Movies

I look at it like the Agents are a filter.
If someone who is not the One enters the source, it causes a problem and might destroy the Matrix. In order to make sure he/she is the One, the Agents must put the person through a series of tests. If they are not the One, they die. If they are the One, they live. Then, they may enter the source.

Answer this one. Why is there no driver in the car that the agent smashes?

Also, The Oracle never stated that Neo wasn't The One. He says something like "So I'm not The One?" and she replies "I'm sorry, maybe in another life", possibly meaning that she's sorry that he is the one.

Maybe Neo could revive Trinity because of their special connection. I have a question how come when Trinity or Morpheus asks him questions about who told him things he evades?

Here's another hole. How do all the Agent Smiths get into the backdoor hallway without any keys? Throughout the movie, it's shown that a key is needed, but they don't have any.
