The pressure of trying not to sound or be like '89 Joker, the pressure of constantly using the extreme intensity needed to play a character like the Joker and the pressure by fans had to of taxed his mind and body severely. But I still say that it partly may've had something to do with it.
He may've had other serious issues.
I think it was only challenging, because after Jack Nicholson he had some pretty big shoes to fill. If no one had ever portrayed the Joker on the big screen, the pressure wouldn't be as intense.
And even if the role did get to him, why wouldn't he have died during filming? Principal photography ended months before he died.

True, it did end months ago.
But maybe the effects had mid to long term effects physically and mentally/psychologically.
Maybe he developed a potentially traumatic biological pattern that caused some damage in the long run. Like for example, he didn't sleep alot while filming TDK, i heard. Couple that with how intense he played the Joker. Maybe his body somehow got used to not really sleeping which in turn affected his brain/mind/body, forcing him to take (alot of) chemicals, to assist.
He did say, if i'm correct, that ever since TDK he's been exhausted physically but his mind just kept racing, or something like that.

Stupid housekeepers/masseuses. They could've checked in on him. Within a 3hr. timespan they should've/must've passed by his bathroom/bedroom area or something and saw the mess of pills with him strewn out at the foot of his bed.
