Virtually all Defensive actions cost exertion, yes (else why not use a basic dodge? There has to be a trade off). I agree it is impractical, speed wise, to take them all the time. it is down to risk judging.
It makes logical sense- all exertion is is a track of how much you are trying to do at once. Actively dodging is always more ffort than just trying to keep a low profile etc.
Ok, two assault rfile attacks on each., and a shotgun attack on all but Azrael. Heph already had his assault rifle attacks though.
