Technically, the Zetsu clones he hit with Kurama's chakra never died, seeing as they became trees, while the Path bodies were already dead. And if Nagato somehow counts for whatever reason, then so does Haku.

All I can think of are the snakes he busted in the Forest of Death, but those weren't people, and the Itachi clone that Pain created.

And a shit ton of White Zetsu but presumably they don't count.

It was unintentional to kill *that* guy, but the giant rasengan to the face was still likely intended to be fatal, just to Itachi instead.

Well if you qualify killing as a loss of sapience it would count.
Contrary to popular conception none of the Big Three show any real outward qualms for killing. The writer just doesn't have them do it because it runs the risk of making the hero look bad.

For all intents and purposes, they are dead.
Their sentience is gone for sure and there is no way to determine if they need to be dead for the technique to work. Is it possible? Sure that might be how it works. Pain does do the whole transferring chakra into corpses thing. But he can do it to living people too if his interaction with Naruto is any indication.
Mind you none of this is an indication that Naruto has some moral code against killing. Seemed pretty damned pissed that it wasn't Itachi. I assume this was what Psycho wanted to ascertain or maybe he was just curious after looking back.
If we discount Naruto killing Yura then the only Big Three protagonist to kill a person would be Ichigo.
