There are only two ways to get killed in my games:
1. You are downed in combat against an enemy that is seeking to kill you and it becomes impossible for you to be rescued by the others
2. You take a dangerous course of action that I warn you in advance is potentially lethal, and it cocks up.
In theory a major lightsabre duel loss is a kill as well, but generally with players we start taking limbs instead. However, come the end of the campaign, which will very possibly be the end of all play, we might take it to the death.

Generally speaking, though, if a character really does get badly messed up, we tend to represent that via things like long-term stat loss. With the style of game I run, actively killing characters tends to harm the story more than is worth it.
