In a non-PIS fight Thor would win every time by just teleporting Hulk into a black hole or something. But comics have PIS to keep them entertaining; so Thor instead stands there trading physical hits with no significant speed difference.

There's no need to misrepresent facts. I'm going to give you the benefit of the doubt here that you just don't remember what actually happened.

Loki-amped Surfer was beating Thor easily. Yes, Thor was holding back, but neither one of them were sure they should be fighting. More importantly, there was plenty of PIS, as neither even used any of their more hax powers.
Surfer with Beta Ray Bill, and Adam Warlock. Thor shits on all three of them. And keep in mind, Surfer proclaims to the world on panel, in big ass capital letters that he's no longer holding back, so there goes that argument.
PIS is PIS, not matter what characters say. When Superman says he's not holding back, but doesn't use superspeed, what do you believe, his words, or his entire publication history showing that it’s in his powerset?
Thor is mortally wounded, and Surfer has a supposed "upgrade". What happens? Right again, Surfer still can't get the job done.
You mean when Galactus comes for Odin? You did notice the whole time that Surfer didn’t actually want to fight, right? And when he saw Thor’s injury he actually lowered his guard out of concern?

The fight was stopped by Sif before they actually got serious. And again, as usual, the plot had them trading their basic atacks, not using their whole powersets to their fullest.
Also, as for the whole "Warrior's Madness not being an amp" argument, that is debateable. But a side issue.
If you look around the internet, you'll see that "Warrior Madness Thor" is always treated as a unique version of the character for the sake of VS threads.
Bill tells Surfer Thor draws strength from his madness--in fact, Bill beats Thor one-on-one at first, then Thor talks to his hallucination and gets up suddenly strong enough to one-shot Bill and easily beat Surfer.
Compared to his entire history, where Bill and Thor are pretty close in power and Thor and Surfer always stalemate, this one-time performance is, if not an amp, at least some sort of outlier. It's definitely NOT his normal outing.
Some other evidence includes Warlock stating that Thor is more powerful than he remembered from their last fight, Moondragon (with the Mind gem) saying he is invigorated by his madness; and Thor later stating that "berserker rage"(which is how non-asgardians would have called warrior madness, according to several previous comics) makes him 10x stronger.
I don't think any of this is truly definitive, especially since Odin calls what happened to Thor not "true" warrior madness, with no real explanation. But the general consensus among fans seems to be that it qualifies as an amp.
