This is the current scuttlebutt on the eye thing
The eye shot is from a vision that Anakin has of his future. see Sleepless Knight. Anakin sees himself in the midst of death and destruction and then when he gets a good look at himself during the vision the red, Maul eyes tell him that he has turned to evil.

jp-but it looks like hes on mustafar in that shot, or is it....oh a vision into the future i get it. so you think that he has this early on in the film? it would make sense for him to see some of the battle between him and ob1. he might get a sense that hes going to die soon as well so he freaks out and starts killing jedi.
