That's because in the original manga they are girls. In the anime they changed it so that they're disguised as guys while on Earth, but change to girls when they transform into their senshi form. It has nothing to do with being gender-confused, and if you knew anything about Sailormoon you'd know that.
Maybe I should make a comment here...
Hentai is not just about perversion or obscenity. Rather certain hentais are to illustrate the beauty of a naked and purity of a character. And also to express the talent and devotion the artist holds for his characters. The human body in a way is a form of art. And the artist involved in hentai seek to bring that art into their work. Of course this doesn't mean that all hentai artists think the same (some might have a certain porn perversion) and use hentai just for profit.
