No it's an alternate reality Spider-Man, but 616 Diamond Lil.

It's real I suppose, it's when 616 Alpha Flight was jumping across dimensions and alternate realities to try and get back to the 616 universe.

edit: 616 Spider-Man has interacted with 616 Alpha Flight in a few issues actually. Just not in that issue, it was an alternate reality version of him.

Just his marriage, if you read the entire issue you can already see that he will be with his Uncle and lot's of other Spider-Man related charcters who died.
