^ New Avengers #27. Dr. Strange is doing what he thinks is best to combat the Incursion threat. They are in an alternate universe sort of, yes (the space between universes). But these are not alternate universe versions of Thor and Dr. Strange. They are the 616 characters. And their campaigns are waged across the Marvel Multiverse.

Man I really hope not. the moment i heard the rumor of a reboot, I thought that it was such a horrible idea. DC had to do it because, well DC writers tend to mess things up so bad every 20 yrs or so that a new Crisis must unfold. With Wildstorm being brought into it, this may happen even sooner. I like DC, but there seems to be too many opinions on how things should be.

He was traveling through a nexus of realities, and the abstract forces that made up the environment, were trying to tear him apart on more than just a physical level (physical, mental, spiritual). I believe that he said that he was being dichotomized.

The point being is that Thanos was being tested on his journey, the scans are from the comic "Thanos Quest #1 - Schemes and Dreams". They were reassembling him as he passed each test of Will power, resolve, and conviction. Thanos at no point during that entire comic book, or the Thanos Quest series manipulate his own molecules and or tangible state of being.
