Based on the art its clear that Mjolnir didn't actually hit him, rather it startled Superman and caused him to bite his own tongue, and because Superman's teeth are so powerful it created a "KRASH" sound effect. Hence the blood. Because we all know from the later fight that Mjolnir isn't a match for Superman's eleventh dial. 😛
