A blood clot in a carotid artery deprives part of the heart from oxygen which results in myocardial infarct (death of heart muscle) if untreated this can cause things like ventricular fibrillation (heart rate becomes a bloody mess) which can effect cardiac arrest (blood isn't pumped properly, so the brain gets insufficient oxygen).
