All the "medicine" is actually doing is increasing the blood flow so that the corpora cavernosa can fill up with more blood...making the penis bigger.
And from what I understand about the penile surgery that they do to make the penis "bigger" is sever a ligament. And all this is supposed to do is make the penis "hang" lower. It doesn't work with some men and on top of that, the erection is no longer "up" any more.
