Likewise he can't just port someone's brain out of their skull because he has to be able to see the target. He can port the entire head of course but that is different from what you are suggesting.
Sasuke instigated the fight sure, but that was only because Orochimaru was going to steal his body soon since the three year limit was up. Sasuke still easily overpowered Orochimaru and easily reversed his strongest genjutsu without even flinching.
Kakashi' abilities haven't been completely explained yet. There are still some holes in their creation, as sometimes things he warps just disappear to never be found again, and sometimes they appear in random places. That's why Deidara found his arm that Kakashi warped. He can obviously control it though, as demonstrated when he warped the Hachibii right in front of the ten tails eye, and when he warped a small nail out of the air as it was coming very fast to his face to kill him. He might be able to warp someones brain out, but as you said, he can definitely warp their head off, which is just as effective.

Indeed. But Orochimaru was still undeniably sick and Sasuke later admits that if Orochimaru had been healthy he would have lost. Also Orochimaru's technique is not a genjutsu. Sasuke simply beat it with genjutsu.
They have been. Deidara's arm was torn off(what Kakashi ported in the anime was explicitly just a part of it that was connecting the arm to his shoulder). If that isn't the case the next person to see his arm was Tobi who can freely travel between the Kamui and real world. If Deidara's arm was sent there(and since Kamui only sends things one way that would be the case) then Tobi could get it back.
As for the Hachibi incident he ported the creature into the dimension before opening a portal to bring it back. If the technique just dropped it randomly then there would be no reason to use it. The nail similarly was sent to the Kamui world and because it was just a simple little nail we don't have to really see what happened to it(my guess is Obito stepped on it and became extra cranky that day).
