They all get a hole in their head. Doomsday, Surfer and Emoboy prime may take a bit longer but they die too.

TBH, I don't know what the f*ck that was.

There are just so many examples of him using high powered HV with no ill effects, that I honestly can't see him being that exhausted from using the highest level, especially when he charges so quickly.
