We haven't seen him pull down any lightning against him and we know that Loki can be easily pinned by Mjolnir. I don't recall Thor tossing Mjolnir at Loki. Even to the end Thor never goes all out against him because he knows that Loki would be hard pressed to survive.
I guess that means Loki held back on Thor since Loki didn't spam blasts at Thor? Make sense. When did Thor get the chance to throw mjlonir at Loki? The fight was up close and personal.

Depends on Thor, the other Avengers are fodder really. Thor would need to fight forum style to beat Hulk, similar to how he fought the Destroyer. I haven't seen any single attack from movie Thor that I'd be convinced would drop Hulk long enough to get the win though.

General vicinity can obviously be massive. As evidenced by his lighting forking out and hitting multiple targets all at once. There's also tornados and the fact he basically one shot raped the Frost Giants' home.

Thor never once missed anyone he fired at, including people that are smaller than the Hulk, if you want to prove the Hulk can dodge lightning, start providing examples.
