Old Kai told him to transform and then he said "ok". After powering up he still looked the same. Plus he was getting raped by Buutenks and didn't think change. Really seems like he lost the ability since all of his potential was unlocked. Basically he's always at his peak stage. At least that's how i took it. Tho you are correct, it was never outright stated
Old Kai told Gohan to 'gather energy like he did when he transformed into a SSJ'. After doing this, Goku speculates that there may be a way to improve upon the form, given that Gohan is still in his base level... But that's neither here nor there.
I believe Gohan could have transformed into a SSJ/SSJ2. However, doing so wouldn't have made him any more powerful as ALL of his potential [transformed] power became accessible in his base level. Essentially all transforming into a SSJ would have done is changed his hair color, and put more strain on his body. /shrug
Because the last time he was awake Frieza was the strongest being by far. His power level was 120 million, second* strongest around was probably Ginyu with 120 thousand. Besides something like Buu who was out of the running. Aside from that being in the tens of thousands was regarded as extremely powerful.
*Cold was around Frieza's level (?) So he's tied first.
Frieza did possess upper-echelon power. I know it's easy to forget this fact, given the ridiculous levels of power we saw in the Android/Cell/Buu arcs, but prior to the Namek arc, Frieza was the most powerful mortal character in the mythos, by MANY orders of magnitude.
Remember, even his BASE form(PL=530,000) was still several times more powerful than any other mortal being in the universe. In fact, the closest PL to his own was likely Ginyu's(PL=120,000)--the rest of the universe's 'high-end' powers were around the 30,000-40,000 mark *at most*. So yeah, that's what Beerus knew/remembered when he went into hibernation 30+ years ago. I imagine it would be very shocking to learn that in such a short amount of time, multiple Saiyan powers had arose that made even Frieza's FINAL form(PL=120,000,000) look utterly insignificant by comparison.
The fact that Frieza has been used as a gauge/benchmark in EVERY arc since, should tell you how powerful he was. 👆

As for Cold's level: Daizenshuu 7 states that he is "somewhat inferior" to Frieza--implying that they are around the same level, but Frieza is still more powerful.
