I think you can only fill it in close to departure because it asks if you have been in contact with covid and that could obviously change in two weeks.
Last year we of course said no but we had been talking to our neighbours and within a week of us arriving one of them died from Covid and another a few days later. So they must of had it when we were talking to them but they didn't know it and we were more than two meters away.
