Spider sense doesn't work with the Spot.
It works on the Spot. The problem with Spot was he would put up many holes from the same dimension so the punch was literally coming from anywhere. Spider sense couldn't focus on where to dodge to when there's only one objective setting it off.

This happened?Is it recent? Series?
It has indeed happened. Albert can show the scan if he wants (I showed it to him lol).
As for the Spot - no, Spidey specifically says that until Spot's fist pops out of the portal, it's like the fist doesn't exist
Because it's in another dimension. Like a Boom Tube.

That's not it
It sets off spider sense it just the dimension itself is doing it and spider sense can't hone in one the one thing when he can create many:

@Stilit: healing factor? Or more likely, writers not knowing/caring, lol.
Yeah, the latter is more likely.
Though they do like to add healing factors to characters' powersets... Green Goblin is a nice example.
Even Bushwacker has one now:
The most recent example would be Pak's Warpath ("Warpak", lol), whose regen impressed the scientists responsible for the Hulkverine project.
Blink ports things with her mind (she stomps), Nightcrawler telegraphs his teleportation (he gets wrecked often because he's predictable and there's an implosion of air before he appears), he cant predict spot...solo is meh. Anyone have any examples of Spiderman getting the best of a good teleporter?
