Remember when you needed a medic, like Rin, in order to transplant eyes? No more, all you have to do is pluck it right into the socket.
Hopefully this is explained with Hashirama's regenerative abilities.

No, he had no eyes at all. That's why he wanted to take Sasuke's, which he would use until he regained his own/Izuna's eyes.
There's a reason they're called the Tailed Beasts!

He did attach Zetsu's arm to his own body in a fashion similar to his eye replacement.
true he did do that indeed. so he must also possess some regeneration ability of his own if his arm stump can so readily accept Zetsu's arm, though it's clearly not powerful compared to what edo tensei shinobi had.
and sage mode or not, you'd think getting smacked around like that would have done a hell of a lot more damage....

Lol, the astounding ease of eye replacements in this manga.
Madara can heal with ease due to his Hashirama powahs.
Replacing eyes has always been easy. Rin put Obito's into Kakashi's eye without much trouble and she was 12-14 at the time. Seems like an easy medical nin technique.
Also, yes, I think his Sage mode saved him from dying when he was getting smacked around. Remember, Naruto fell dozens of feet straight down on to some stalagmites that he was sure would kill him. He pulverized them when he hit them. So Sage mode offers a stupid high-damage resistance buff. Madara was damaged fairly badly even in his sage mode. So badly, in fact, he lost an arm.
yeah, but even then it seems like the boost to durability that Madara got from sage mode is seemingly much higher than the one Naruto gets from his. seriously, it took getting smacked around by nearly all of the biju plus a big puddle of acid and nine simultaneous tail smacks just to make him lose his arm.

Yet Sasuke was able to stab through Madara like butter. So, Sasuke's sword>>>multiple biju strikes?

That's a good point.
Naruto also seemed to get stabbed through very easily by Pain's rods even when in sage mode, iirc.
Seems sage mode doesn't do well against metal blades that can be infused with chakra (lightning or 6-paths communication chakra)?
3 things:
madara's borrowed eyes vanished cause they belong to a corpse according to plot cues. his true eyes are both still living (i assume obito took them both from nagato's corpse and stored one in a jar)
a well respected reviewer commented on a possible (and epic as phuck) stratagy used against the alligence that would make this war T1TS
Spoiler:
madara has the black rods in hashirama's back, so with the rin'nigan he could control hashirama like a path, that means you get to see all his crazy power, used on all that fodder, same can happen to the other edo KAGE
the episode that just dropped was A1

wakkawakkawakka: maybe people in sage mode are really strong against blunt attacks, but ones that pierce them like swords are their weakness? only thing that makes sense to me right now.
I thought of that, too. Then I remembered that Naruto fall from many meters up into some stone stalagmites and he pulverized them instead of being impaled.
So my next conclusion was it had to be only those blades that could be chakra infused.
Nagato's chakra rods and Sasuke's sword are both special metal blades that can handle lots of chakra. So, my assumption was only those special chakra blades can easily penetrate a sage mode user.
However, the real reason is simple: Kishimoto f*cked up.

On the susano'o, I wonder if it's something like, while it's born of the jutsu of the two eyes, once you have it, you have it and it's in the body? It's not like it's shown coming from a specific eye like the prerequisites.
Susano'o has been activated/sustained a handful of times when the user did not have the MS/EMS activated. While Sasuke states that having the double mangekyou is the requirement to obtain Susano'o, we aren't told that it's needed to use Susano'o.

And of course there's Madara with his Susano'o shenanigans.
