No.

As far as I know, no.

Except in KOTOR where it seems to happen all the darn time.

I remember that Ulic Qel-Droma had his connection to the Force severed by Nomi Sunrider, which is where KOTOR got the idea. He still could use some things to a degree, I think...he disappeared on death.
