How do Yoda and Obi-Wan KNOW Vader is Anakin?

The answer to the question is, despite the way new Sith turn up at the drop of a hat, who else could this new guy be? Anakin is a safe guess, I'm sure.
Come to think of it, though, hasn't it been said that it would be made clear that Anakin is Vader?
