It's set after the events of WWH. In the book Banner seemed to already know a Red Hulk was running loose when Iron Man asked for his help but he was still locked in his underground cage. So it was sometime after Rulk assaulted Iron Man and the helicarrier in Hulk #2 and prior to the Rulk/A-Bomb fight in issue 3.

So in other words, it was Rulk hopped up on Loebforce
