He is sealed in the Crimson Cosmos, but an aspect of himself is sealed inside the Gem and that's what grants Cain his powers and makes him so destructive for the sake of being destructive.
And the reason he predates the other 7 Exemplars is because apparently Cytorrak's temple collapsing interrupted the process of the Octessence. It's also why Juggernaut more or less had free will, because he'd gotten use to the power so long before the other Exemplar's emerged. And there's only 1 Juggernaut, the other 7 and their gods are :
Bedlam - Ikonn
Stonecutter - Raggadorr
Tempest - Watoomb
Decay - Verdant
Inferno - Balthakk
Carnivore - Farallah
Conquest - Krakkan
Also, Juggernaut was further weakened after helping stop the Octessence plot.

And Cain wasn't the first Juggeranut as we came to find out in New Excalibur. There have been an unspecified number of them before Cain came along. I can only think of two at the moment but the Juggernaut power has been here and used since ancient times and their main purpose is to use their powers to spread and cowtail people to worship Cyttorak.
I think when the bet came along Cyttorak just used his avatar of destruction in the bet.
The main thing though is we know for sure they magic based entities and odds are that is why the Exemplars were able to hurt Cain physically considering we know what Cain can take.

Oh, I misunderstood you when you were talking about the "Juggernauts". I thought you were referring to the other Exemplars, not the Juggernaut power itself. You're right on that.
