Bruce has anger management issues, but as of TIH (2008 movie) he can consciously control his temper and keep it from rising to the point where he transforms into the Hulk. Whenever he wants to transform he just stops controlling it.
The reason why he transformed and attack Black Widow is because he was already pissed due to the argument he was involved in a few minutes earlier, and that combined with the trauma from falling through the floor and being pinned by debris riled him up to the point where he couldn't focus enough to control himself.

Ruffalo probably had the best performance in the movie though, I'd say. "I'm always angry" IMO says just what it sounds like: He's always angry, and is constantly struggling to control it. There is a subtle undercurrent of anger in Mark Ruffalo's acting that really conveys this IMO, it was an excellent performance.
