Hulk wasn't knocked unconscious. Oxygen deprivation eventually got to him. Banner controlled Hulk isn't as durable as Savage Hulk.
Regardless, the issue doesn't involve Savage Hulk.

Surfer wouldn't try to kill him. That's out of character. He'd use enough force to defeat him, and no more.

A knockout is a knockout? Its 1 vs 3? Whenever odds are unbalanced like that, the individual is always gonna be slightly overpowered.
The cage was a simple construct.
And yes Hal DID survive.
Id rather get knocked out by Thors hammer than my Nita(Namors cousin)
