I actually got it in-game, when you get to the second room of the mansion (main hall I think it is) there's like 10 grunts to fight there..
the problem most of the time is button mashing. I'm pretty sure you will always counter as long as you pressed "Y" in time and you haven't pressed any other button before it.
the key is to time button-pressing..

It seems most people get the impression this is a button mashing game.
Its easy, you just need good timing.
Press block right away if someone shows a spark above their head.
Jump around to cause downtime so your enemies get up faster so you don't punch someone thats already on the floor which ends up losing your streak.
I usually punch a guy three times, jump over the person thats about to attack me, punch him two times, thats 5 points right there to use a combo on another guy to take him down real fast
