He did. But canon doesn't grant Nick Gillard G-canon status or special authority. Is it possible (and even likely) that that was what was intended for the Emperor? Sure. Especially since no one bothered to contradict Gillard. But the fact remains that I do not debate intent from individuals nor is Gillard a canon source.

However, if what you say is true, then my 'Bane mastered all seven forms' would be very incorrect due to the premise being wrong, and I'd concede.

But Bane did know all of the sequences of the forms, thats how he fights them, by memorising sequences. Its extremely pluasable that he could fight and use the other styles, but I would hesitate to call him a 'master' of them.

Is there actually proof that Sidious personally trained Maul or that he was otherwise a Master of Juyo?

The same reason that Anakin Skywalker could mimic stances and engagements of multiple forms, though nothing suggests he'd mastered any more than one.

Well, obviously. Everything about Sidious is ambiguous, Tangible, even though he's the most explored Sith Lord outside of Vader. You didn't know that that ambiguity also taints Maul's training?

Sorry, Veneficus, but if you witnessed as much from Taven (Nebaris) as most of us have, you'd jump all over him too. He revels in nitpicking. And not the good nitpicking either.
