It depended on the novel and story. In the beginning, there were tons of questions and directions given by GL regarding those things.
Later, though, after "they" had a better feel for what GL would allow or not allow, the questions subsided. If there was a sensitive story that came up, he would do the grilling and veto stuff, again.
