I do not know who added the millionaire part to his insult but TE's assertion about it being just slum dog seem to be correct, thus far.

Fact is both India and Pakistan have a state/province called Punjab. Fact is that in both these Punjabs, punjabi is the main language taught as opposed to Hindi or Urdu.
Suffice to say, the #3 definition doesn't even come close to being a racial slur.
