All of creation singularly exists through his will. So it takes up much of his time, his mental capacity, and really everything.
It's one of the reasons why Doctor Strange is so important and the reason why their partnership works. This absolutely was them pulling everything that was left and making something where there was nothing. Doom relies on Strange intimately to handle the day to day running of the world. You could argue they desperately need each other.

It could prove interesting because what with Stephen now dead and Doom wont be able to run everything perhaps he might lose control. The cover to the next issue could perhaps be a hint of what's to come, who knows?
Next issue is going to expand on what actually happened between Doom/Molecule Man and the Beyonders.
Hickman: Next issue you kind of find out what happened between Doom and the Beyonders, and what happened to the Molecule Man, and all that kind of stuff. So it's an issue about the mechanics of how everything works, and then what happened to all of the life raft characters. So there's a lot going on.

They became part of the Beyonders expirement.
I'm pretty sure the cover is just showing a load of key events/characters from the MU. Although I have no idea who the parents and baby in the very top left are.
Maybe the silhouette is Molecule Man? I have a feeling he might be the one to help rebuild the multiverse- with Reed and Doom's knowledge and help perhaps?
