As for regular goons, they failed to shoot Kingpin at near point-blank range and he blitzed the shit out of them without much effort on his part.

Just because the end of the Ik/Th fight was an illusion, doesn't mean the rest was as well.

We see battle damage on Ikaris and he mentions his hands being stained with Thanos' blood, too.

It needs to be said that Thanos looks like he was anally raped before their fight, though.

It's pretty obvious the illusion was created solely for the end of the fight to feign the decapitation.
