You can tell Earth 1 and 2 Batman apart from the yellow ovel around the bat symbol. Earth 1 "mainstream" Batman officially had that (I never cared for it, and am glad its long gone)

The Cosmic Ghost Rider, as the Spirit of Vengeance cannot die apparently except for Mjolnir which has a history of bypassing mystical enchantments that were basically impossible to break. 🙂

no way to know, it was written 20 years ago, back then DC cosmology wasn't this complexed, so take it for what you will.
the way the writer worded it, seem like Mxy destroyed "everything", whether it's the entire multiverse or megaverse/omniverse, we dont know
