Depends on how recent the complaint was. In Bleach Kubo made it look like he was killing tons of people in the recent war...until it turned out to be a lie.
Yes. I was really surprised anyone thought Byakuya was dead after it was just, "Mortally wounded and his reiatsu disappeared."
A condition of which *no-one* has died in in the series, and is exactly what happened with Rukia vs espada 9.
He's done the fake-out thing multiple times too, like during the fake town/deicide arc (get cut in half? No big deal!).
In Naruto, most deaths are real. There was one notable exception wherein characters 'died' but were returned, though that one did have the return-er die, and additionally to that there've been cases where people have serious wounded but get medical care in time and no-one pronounces them dead.
There's been a good number of real character deaths in Naruto. Sarutobi, Jiraiya, Asuma, and the recent ones being the most notable.

Two occasions actually. Chiyo resurrected Gaara.

Yes. Every time actual deaths were undone, it involved other characters dying. One-on-one the first time, while the second time got a better deal.
With death being a major theme in Naruto, it isn't surprising to see Kishi killing off some of the good guys to be honest.

Naruto will create Love/Friendship chakra mode and bring her back.Their love will persist.
Naruto feels nothing but annoyance at Hinata's uselessness. He intends to die with Sasuke. It is that love that will persist.

Is Kabuto still getting mind phucked?
Itachi was the one who cast the jutsu no? 313
he was sort of coming to terms with himself in his mind the last time the genjutsu was shown.
i wouldn't be surprised if he didn't pull an orochimaru and return (oro did it twice after supposedly dying for good) cause you all know you can't just leave a guy like that by himself to what..die of old age? no, we will be seeing kabuto again
