How come their eyes are fine from such repeat usage while Sasuke was going through the bleeds, suffering eye pains and was already losing his vision in a month or two? There's no way he's used it more than them in such a short time.
Then again Itachi has had his for a several years with obvious use and his eyes only recently got to a unmanageable state.. hmm..

Kakashi doesn't really use it much. Neither did Itachi. Both used it sparingly, unless it was absolutely needed. By the time of his death, Itachi had had his MS for almost a decade, and this is how bad his eyesight was before the fight started. Kakashi unlocked his a couple of years ago at best and had only used it a handful of times (Deidara, Pain, Sasuke, and maybe Hidan/Kakuzu?) prior to the War arc where he started spamming it.
Half of Obito's body is made up of Hashirama's cells. We all know how hax those cells are. That is presumably why his eyesight hasn't deteriorated much, if at all.
Amaterasu is the only MS tech that makes the eye bleed. Sasuke spammed MS techs in his battles. His techniques against Danzou were almost exclusively MS techs. Maybe using it in succession heightens the rate of deterioration, and maybe being low on chakra while using it heightens the rate as well. srug
In summary, Kishi needed to advance the plot and power up Sasuke by the time of the War. 👆
