My guess is that Real-Madara did something before he died at VotE that led to the creation of Tobi-Madara. Tobi-Madara then gave Nagato the Rinnegan, and their plan was to have him use the Rinne Tensei to bring back Real-Madara. After all, Real-Madara assumed he had been resurrected via some other technique when he first stepped out of the casket, and the only logical answer is Rinne Tensei. As to how Real-Madara knows about Nagato...I'm stumped.
That said, Real-Madara appearing out the casket was lulzy. Everyone else had the door slide out. This guy kicked it down.

He is way too old to have been around during Nagato Uzumaki's lifetime. So either he didn't die at the VoE(and he is in the armor and everything from the time) or he had an apprentice named Nagato, who had the Rinnegan and sharingan, i.e Tobi. I say apprentice because the Kyuubi recognized him and there are so few people the Kyuubi should recognize.
If this is right, no brainer what the Sages real name probably was.

Well I see it as sort of a good thing really. So long as Kishi does plan to kill one soon. Remember, they're fighting a war against people who can revive the dead. If a named character, say Neji, were killed, he could be brought back. This would lead to some potentially cool moments but after a while it would lose weight.
