Thoughts:
Batman is near the same level as Captain America physically. Batman is an olympic level athlete. The peak of human performance. BUT Captain America is beyond normal human levels. He's a Super-human thanks to the super soldier serum. Captain America doesn't get tired. If they were to fight purely hand to hand, Batman would eventually lose. Batman even admitted it in JLA/Avengers. It would just take captain america a while, but eventually he would wear Batman down.
But what Batman does has the edge in, is in the gadgets and prep-time department. Batman's got a ton of gagdets at his disposal. Captain America just has some Avenger combat gear and his good ol' trusty Shield.
With those thoughts in mind....
With preparation time specifically designed to take down captain america, the batman takes it 7 to 8/10.
Without prep time, and just his regular standard gagdets Batman takes it 5 to maybe 6/10
With just "hand to hand" and no gagdets or shields for either side, then Captain America takes it 7 to 8/10.

I honestly wouldn't say that Cap's physical advantages are so high over Batman's to say that he'll win simply because of them. Ok, he's Batman's physical superior in every aspect and is (arguably) a better fighter, but Batman is olympic level and one of the top fighter in DC, but he also the edge in his gadgets. Batarangs, gases, flash bangs etc will all throw of Cap sufficiently for Batman to score a win. Straight up fight like this Batman 6/10.
But like Evangel said, no prep or weapons on either side then Captain America 8/10.

That a load of crap. Using your logic batman should be dumber than cap, because batman uses gadgets alot. The only gadget cap has is his shield, and he even then he can still be effective without it.

Cap is stronger he picked up Big Bertha and toss her like a baseball, srenght they are not even, Azrael is just as strong as Cap and he serve Batman to the point he had to save Batman. Cap did trained in the future in the Korvac saga and he retained that fighting abilities, in pure h2h combat the edge goes to Batman since Cap is more like a grappler and then a striker. Cap 10/10

even with prep batman can still lose, Cap is the perfect human as batman is human with great abilities. Captain America will push batman to the breaking point, Batman had prep on deathstroke and still got his head kicked in. Cap is one the best h2h combatants in comics, 10/10
