Probably teleport him over a large body of water, hitting water from two miles up is probably as bad as hitting ground, adamantium bones sink, therefore asphyxiation follows dealing with that pesky healing factor.

Actually, there's already record of Shadowcat fighting Logan almost for real, and that schooled him enough. He taught her just about everything he knows, and then she went and developed her very own martial art that utilizes her phasing ability, making her seriously one of the deadliest martial artists on Earth. She schooled Logan and she was under mind control then. Think about if she did it using her own judgement? She would mess him up something nasty.
She can phase just fine through adamantium.

Yea, that's when it happened. She still does have the ability to unphase certain parts of her body whenever she wants. It's CLEARLY OBVIOUS (I use caps to emphasize to non-believers, not you, heh) that she can, since most of the time when she phases, she's still standing on the ground? Eh? Eh? If she wasn't able to phase certain parts, she would always fall through the Earth's surface when she phased.
Kitty can phase just fine through adamantium. She can also solidify her hand inside Logan's heart, killing him almost instantly.
I don't think she's allowed to solidify inside of things, though if there's space which there is she could selectively solidify her her hand around it (her wrist through the pectoral and ribcage would have to stay phased). Thing is when she's phased she 'floats' on air so she may or may not have her feet solid. She may just look like she's standing on solid ground.

Momentum doesn't work like that, he doesn't instantly gain a huge amount of momentum as soon as he ports up. He needs to gain velocity first in order to gain momentum. If he teleports away a second after (I think he can do it fast than that anyway) he's only gained a speed of 10m/s which is manageable for a master acrobat like Kurt.
Wolverine however will hit terminal velocity, which with adamantium bones is very high velocity from 2 miles up.

Kitty can't phase through adamantium "just fine," because of its desity it cause her alot of pain and almost knocks out anyone she takes with her in the process

That was strictly need to advance the plot which the reason it didn't hurt Kitty.
The only issue I know where it is shown is where Wolverine is about to merry Viper and Sabretooth shows up but it was established looooong before that in an Uncanny issue.
